Notes on Implant-Retained Overdentures

Although implant-retained overdentures have been hailed as the standard of care, most dentures today are still conventional tissue-supported prosthetics. Why is this? One important reason is that many patients are uninformed about their options. The OA recently sat down with Dr. Mohamed Attia of Alexandria, Virginia so that he might keep us better informed – especially on implant-retained overdentures utilizing PreatLoc™. We learned a lot and took a few notes. Here are points for your consideration:

1. For some patients (especially older ones) a Removable Overdenture might be a better option than a fixed prosthesis and certainly a conventional denture. “It’s more affordable, easier to clean and maintain.”

2. When explained effectively, a suitable patient will likely desire stable dentures utilizing an overdenture attachment system.. As the saying goes, “Where there’s a will, there’s a way.” If patients can’t have implants on both arches, they may opt for at least 2 on the mandible for that irritating “floating” lower denture.

3. It is recommended that new dentures be made first, then retrofitted after implant placement and osseointegration. This gives the patient a denture now, and more stable denture in the future. Care should be taken to ensure intaglio surface is adequately relieved around healing-cap or abutment until fully integrated.

4. When ready, “Direct” placement of the PreatLoc™ attachments is extremely easy utilizing chairside acrylic. This saves on your lab bill and doesn’t require your patient to be without their dentures.

5. Ensure adequate material has been removed and verify denture is fully seated (with attachment on abutment) prior to “chairside pick-up.” Take note of how denture seats and teeth occlude.

6. After attachment housing has been added, always start with the least retentive insert (blue, then pink, then clear).

7. Don’t rule out attachments for partially edentulous patients. Dr. Attia referenced successful outcomes where it was best to retain healthy detention while still utilizing implants with PreatLoc™ for optimal retention and esthetics.

8. Keep patients coming back every 6 months to 1 year for routine check-ups. You can check if inserts need to be replaced at that time (no acrylic needed for this step). It’s a snap! Get it? See what we did there?
